Incident review: IPC Partnership with an NHS healthcare organisation

The challenge

A large healthcare organisation experienced a significant infection-related incident that placed considerable pressure on clinical, operational and support services. The incident evolved rapidly and required coordination across multiple teams, including Infection Prevention and Control (IPC), microbiology, emergency planning, clinical services, communications, senior leadership, and external stakeholders. Following the event, the organisation sought an independent external review to provide assurance, identify learning opportunities and strengthen future preparedness and resilience.

The approach

IPC Partners undertook an independent review of the organisation's response to the incident. The review was conducted as a learning and improvement exercise, designed to understand what worked well, identify opportunities for enhancement, and provide practical recommendations for future incidents.

A structured methodology was used, incorporating:

  • Review of relevant policies, plans, governance documentation and incident records.
  • Assessment of clinical, operational and organisational response arrangements.
  • Engagement with key stakeholders from across the organisation.
  • Analysis of strengths, risks, lessons learned and improvement opportunities.

This process enabled findings to be gathered across multiple sources of evidence, supporting a balanced and objective assessment.

The solution

The review examined all aspects of the organisation's response, including:

  • Clinical pathways and patient management.
  • Recognition, escalation, and incident management processes.
  • Governance, leadership, and decision-making structures.
  • Communication and stakeholder engagement.
  • Information management and reporting processes.
  • Workforce resilience and organisational capability.
  • Preparedness and emergency planning arrangements.

IPC Partners synthesised the evidence into a comprehensive review report, highlighting examples of effective practice and providing a prioritised set of recommendations to strengthen preparedness, governance, resilience and organisational learning.

Impact

The review provided senior leaders with independent assurance regarding the organisation's response and a clear evidence base for future improvement. The findings recognised the professionalism, adaptability and commitment demonstrated by staff while identifying practical opportunities to strengthen systems and processes.

The resulting recommendations provided a roadmap for enhancing organisational resilience, improving incident management capability and supporting a more coordinated response to future infection-related challenges.

IPC Partners provided a thorough, objective and constructive review. Their ability to engage stakeholders, understand complex operational issues, and translate findings into practical recommendations gave us valuable assurance and a clear framework for future improvement.
